Inclusion Health: Integrated Care for Homeless

Dr Nigel Hewett will be talking on Thursday July 20 at 6pm about Inclusion Health and how health systems can respond to improve the health of excluded groups, including people who are homeless, vulnerable migrant groups, and marginalised populations. He is the Medical Director of Pathway – a Charity formed to improve the health care of homeless people and other excluded groups. There are now 11 Pathway hospital care coordination teams from Bradford to Brighton in England, and the Pathway model was one of the inspirations for the Homeless Team model established at Royal Perth Hospital in 2016. Given the recurring media spotlight on the escalating health budget in WA, his visit is timely as the Pathways model has demonstrated significant reductions in health service use through more integrated and targeted care pathways for people who are homeless.
